Patient Appreciation Discount Plan
No Insurance? No Problem!
Who is eligible?
Any patient who does not have dental insurance.
This is not an office insurance plan, this is however a discount courtesy to our valued patients. If a patient maintains their yearly (12 month) fee, the courtesy discount will always apply, however cannot be re-instated if it is not renewed each year (12 months). Patients must have or already had a complete oral evaluation with full set of x-rays before purchasing appreciation plan ( not included in appreciation plan price).
Premium Appreciation Plan
For an annual fee of $479 & no enrollment fee (for patients who are new to the Premium Plan).
What is Covered:
- 2- adult cleanings
- 4 Bitewings
- 1 – 2 yearly exams by Dr. Evans
- 1 problem focused exam and x-ray
- 10% discount on any treatment needed
This is a savings of 30%!
Premiere Appreciation Plan
For an annual fee of $579 & no enrollment fee (for patients who are new to the Premiere Plan). This is recommended for patients who have Periodontal Disease that leads to needing more extensive cleanings.
What is Covered:
- Plan cost is $579.00 per year (12 months) and includes:
- 4- periodontal maintenance appointments per year (12 months)
- 4 Bitewings
- 1 – 2 yearly exams by Dr. Evans
- 1 problem focused exam and x-ray
- 10% discount on any treatment needed
This is a savings of 50%!
